As part of the Federal Governments initiative to improve health care for older Australians the Glen Forrest Medical Centre like to offer patient’s over 75 an Annual Health check which will be bulk billed to Medicare.

As part of the assessment, Anna our health assessment nurse will visit you at home to do a comprehensive health screen that includes looking at some of the following:

– Past history
– Family history
– Allergies
– Nutrition
– Assess home safety
– Assess social support
– General  health assessment check  including: Weight, blood pressure, urine analysis and blood sugar level.

You will then need to make a half hour follow up appointment with your doctor who will then look at anything Anna has recommended for further assessments and implement any changes that may be required.
