What to expect:
As a new patient the doctors would like you to book a long appointment, this is for both the patient and the doctor to have time to get to know each other.
Before your appointment you will receive an sms or email with a link to complete the new patient paperwork online.
If you are unable to complete it online please arrive 10 minutes before your appointment to complete in the surgery, do not forget your medicare card and any concession cards you may have.
When it comes time to pay your account we offer a $5 discount for payment on the day (fees) and are able to help you claim the rebate from Medicare by either putting the rebate back on an EFTPOS (cheque/savings) card instantly or if you have registered your bank details with Medicare we can send it off to them and they will put the rebate back in your registered account within a day or two.
If you decide to become a regular patient at the Glen Forrest Medical Centre you can request your medical notes from your previous practice: Request to Transfer Notes to GFMC Form
